In an Emergency, Call 911
If you are having a medical emergency, call 911 or go to the nearest emergency room. Do not use this form or wait for a reply. Chest pain, trouble breathing, sudden weakness or confusion, and a new severe headache with vision changes all need emergency care.

Frequently Asked Questions
Contacting Our Office
What is the fastest way to get an appointment?
Call our office during business hours, or send an appointment request online and our team will call you back. The online request is not an instant booking. It lets the person who calls you understand your needs first, so we can schedule the right length of visit.
Do I need a referral?
Some insurance plans, including many HMO and Medicare Advantage plans, require a referral from your primary care doctor before a specialist visit. Check with your plan, and our office can help you confirm what you need.
How do I reschedule or cancel an appointment?
Please call our office rather than using the form. New-patient visits are longer than routine appointments, so letting us know early allows us to offer your time to another patient.
Can I email my symptoms or test results?
Please do not. Email and this form are not encrypted or monitored around the clock. Send your name, phone number, and a short note about what you need, and our office will call you to go over the details.
What should I do if I need help outside office hours?
For an emergency, call 911 or go to the nearest emergency room. For anything else after hours, call our office at (832) 305-5226 and your call will reach our answering service.
How do I get a prescription refilled?
Ask your pharmacy to send the refill request to us directly. It arrives with the medication name, strength, and prescription number, which makes it faster than a phone call. Please allow several business days, and longer if your insurance needs a new authorization.
How do I request my medical records?
Records requests must be made in writing and signed. Call our office for the request form, and be specific about the dates and records you need. A focused request is processed faster and is more useful than asking for everything.
Where is your office located?
We see patients at one office: 19002 Park Row Blvd, Suite 100, Houston, TX 77084. Patients visit us from Katy, Fulshear, Cinco Ranch, Cypress, Richmond, Rosenberg, and across West Houston.

Still have a question?
Our office is happy to answer questions before you book. Call us or send a request and we will call you back.
